Care Finder
Tuart Place has been supporting older adults across Perth for more than a decade through advocacy, practical assistance, social connection and specialist support services. Our Care Finder service builds on this experience by providing intensive support to older people who need extra assistance to understand, access and stay connected with aged care and other community supports.
We support older adults across both the Perth South Metro and Perth North Metro regions.
Although Tuart Place is nationally recognised for supporting Care Leavers, Forgotten Australians, Former Child Migrants and Stolen Generations survivors, our Care Finder service is available to any eligible older person who faces significant barriers to accessing aged care and related services.
How We Can Help
Our Care Finders work alongside older people who may be experiencing challenges such as:
- Social isolation
- Trauma histories
- Housing insecurity or homelessness
- Cognitive impairment or memory concerns
- Mental health challenges
- Low literacy or digital exclusion
- Limited family or informal supports
- Cultural or language barriers
We understand that navigating aged care can sometimes feel overwhelming. Our Care Finders provide practical, hands-on support and can assist with:
- Understanding aged care options
- Registering with My Aged Care
- Preparing for aged care assessments
- Understanding assessment outcomes
- Exploring Support at Home and other aged care services
- Connecting with health, housing and community supports
- Advocacy and coordination where additional support is required
People can meet with their Care Finder at our Fremantle hub, in their home, or at another location that suits their needs and preferences.
Our approach is trauma-informed, person-centred and culturally safe. We work at the person’s pace and focus on building trusted relationships that support informed choice and decision-making.
Referrals
Referrals can be made by:
- The person themselves
- Family members or carers
- Friends or supporters
- Health professionals
- Hospitals and health services
- Community organisations
- Aged care providers
- Other services involved in supporting the person
